Whole Health Coaching Program Participant Agreement

This form is intended to provide you with information to help decide whether working with a Whole Health Coach is for you.

What is Whole Health Coaching?


Whole Health Coaching is a personalized, one-on-one experience that helps you explore your goals, values, and vision for a balanced life. Your Whole Health Coach is your partner in identifying matters most to you and supporting your creation of a plan that is both meaningful and achievable.

The program includes a biometric health screening with an advanced-grade scale to provide insight to support realistic, data-informed goals that align with your strengths and lifestyle. Working with your Whole Health Coach to review your personal health inventory and biometric data allows you to gain valuable insights into your current health status. By combining your personal aspirations with science-backed strategies, you can create a tailored plan that supports your journey toward optimal well-being, helping you achieve your health goals more effectively and sustainably.

Whether you want to move more, manage stress, improve sleep, or reconnect with purpose, your coach will help you create actionable steps grounded in both your values and health data. Not sure where to start? That’s okay—we’ll figure it out together.

What is "Whole Health"?


Whole Health is about seeing yourself as a whole person, not just your physical health, but your beliefs, relationships, environment, and more.

What can I expect from my coach?


Your coach is here to:

  1. Listen deeply and without judgement
  2. Help you clarify what matters most
  3. Support you in setting and working toward your goals
  4. Celebrate your progress—big and small
  5. Respect your lived experience and autonomy
  6. Your coach will show up fully for each session, and we invite you to do the same. This is your time.

What is NOT Whole Health Coaching?


Your coach is not a clinician, but a trained guide to partner with you to help you move toward the life you want.

Whole Health Coaching is not:

  1. Therapy or clinical care
  2. Medical advice or diagnosis
  3. A substitute for professional services (legal, financial, spiritual, etc.)

How are coaching sessions structured?


The Whole Health Coaching Program includes six (6) coaching sessions, 55 minutes each (at your own pace) + a 30 minute Program Consultation

-Program Consultation: including a biometrics scan with an advance-grade scale
-Session 1: Personalized Plan Development
-Sessions 2-6: Guided Strategy Discussion
-Biometric scan (Session 6)

What commitments will I be asked to make?


To make the most of your experience, we invite you to:
• Show up on time and ready to engage
• Be open and honest—this is a safe space
• Limit distractions (silence your phone, close your laptop if possible)
• Try out the practices or reflections between sessions
• Let us know if something isn’t working—we’re here to support you


If You Need to Reschedule

Please call the VEBA Resource Center at (619) 398-4220 at least 24 hours in advance. If you’re more than 10 minutes late, we may need to count it as a missed session.

Agreement to Participate


By choosing to participate in the Whole Health Coaching Program, you are affirming the following:
    1. I understand that my health and wellness journey is my own, and I am responsible for the choices I make.
    2. I agree to engage with my coach in a respectful, open, and honest way.
    3. I understand that my coach is not a medical provider and cannot offer clinical advice, diagnosis, or treatment.
    4. I will do my best to be present and prepared for each session.
    5. I confirm that I have completed the Wellness Inventory survey and that the information I shared is accurate to the best of my knowledge.
